On 12 May 2000, HMS Tireless suffered a "Loss of Coolant Accident", involving her main reactor. It was initally thought (and reported) that the problem was a minor crack in a coolant pipe. HMS Tireless arrived in the port of Gibralter shortly after the accident for what was planned to be a short visit.
HMS Tireless finally left Gibralter on 02 May 2001, nearly a year later.
Following the discovery of the crack on Tireless, each of the other 12 vessels of the class were removed from service temporarily, reportedly so that their reactors could be checked for a similar problem.
